Shortly after the engines has been repaired por. pil. Słoński - Ostaja and 
F/Lt. Paul Riches R.A.F.  flew with Mosquito Mk XI, code LR 512 a test-flight.
The aircraft crashed against the "Aran Fawddwy Mountain" near Dolgellau, Wales. 
Both died.

His brothers Słoński-Ostoja Bogdan and Słoński-Ostoja Karol Józef died as well.

On the 3 headstones the family-name OSTAJA-SŁOŃSKI has been mentioned, 
but in Polish sources SŁOŃSKI-OSTAJA is always recorded. 
On the Polish War Memorial at Northolt the name is written in the same way.
